Passport Services – General Information
The Consulate provides passport services for two categories, fresh issuance of passport (for newborns) and renewal of passports for Indian nationals staying in its Consular jurisdiction .
BLS International is the sole authorized service provider for Consular services on the behalf of the Consulate. For info about BLS services please visit the website https://blsindia-canada.com/
HOW TO SUBMIT THE PASSPORT APPLICATION (DOCUMENTS REQUIRED AND GUIDELINES FOR FILLING APPLICATION):
Documents to be submitted with passport application:-
A. Re-issue of passport due to expiry of validity / exhaustion of visa pages
a. Printout of the Checklist click here
b. Printout of duly filled Application Form (available online)
c. Current Indian Passport (Original and Copy)
d. Specimen declaration of the Applicant on a Plain Paper - Annexure ‘E’
e. Proof of present Canadian address (see FAQs for list of acceptable address proofs)
f. Proof of valid residential status in Canada (see FAQs for acceptable documents)
h. Two Recent Photographs (Specification of Photograph can be found here)
B. Reissue of Passport due to change in personal particulars like:
I. Addition/deletion/change of spouse's name on passport on account of marriage/remarriage/divorce/separation
(1) For addition of spouse name in Passport:
A) Marriage Certificate OR
B) Joint Photo Declaration signed by both husband and wife
(2) For deletion of spouse name: Divorce Order/Decree
(3) For change of spouse name:
A) Divorce Order/Decree OR Death Certificate of first spouse
B) Re-marriage certificate OR Joint Photo Declaration by both husband and wife
(4) For change of surname by womwn applicant following marriage/divorce:
A) Marriage Certificate OR Joint Photo Declaration by both husband and wife
B) Divorce Order/Decree (if name / surname change is based on divorce)
* In case of full change of name after marriage, the standard name change procedure should be followed.
II. Reverting to maiden surname on account of divorce or separation
1. Applicant must have an earlier passport with maiden name. No proof of divorce or separation is required in such a case. However, If the copy of older passport of maiden name is not available, please follow (IV).
III. Change of Name (Cases other than B(I, II, III) above)
1. Two public IDs/documents issued by Government authorities.
2. Original newspaper advertisement / clippings of two leading Indian newspapers or Publication of the change of name in the Gazette Notification of India..
IV. Change in the name/spelling of a Parent
1. Parent’s Passport Copy
V. Change of appearance/signature
1. Self-signed declaration for the change
VI. Change of Address
1. Proof of present address (see FAQs for list of acceptable address proofs)
C. Re-issue of Passport – due to Loss/Damage of earlier Passport
a. Printout of the Checklist Click here
b. Printout of duly filled Application Form (available online)
c. Current Indian Passport (Original and Copy)
d. Specimen declaration of the Applicant on a Plain Paper - Annexure ‘E’
e. Proof of present Canadian address (see FAQs for list of acceptable address proofs)
f. Proof of valid residential status in Canada (see FAQs for acceptable documents)
h. Two Recent Photographs (Specification of Photograph can be found here)
i. Specimen Affidavit for issuance of a passport in lieu of lost or damaged passport - Annexure ‘F’
j. Letter explaining how Passport got lost / damaged.
k. Original Police Report mentioning lost Passport number.
D. Re-issue of Passport to minors
(Note: Minors between the age of 15-18 years have the option to submit application for full 10-year validity passport by paying full passport fee of a regular passport)

i. Passport copies of both the parents
j. Annexure ‘D’ or Annexure ‘C’ as applicable (see FAQs for more details)
k. For a minor child with a single parent due to divorce or death, documents such as a divorce certificate or death certificate for the deceased parent is necessary. Additionally, a custody order from an Indian Court is required.
l. In case of case both the parents are residing outside Canada – attested Annexure ‘D’ / Annexure ‘C’ as the case may be.
While submitting application, please keep the documents with the application form as per the above serial order.

Note that processing time starts with arrival of application in Consulate, it does not include postal delays. Applicants are advised to check the status on BLS website at this link .
